Making a successful transition from paediatric to adult care is a very important stage for all young people diagnosed with Ulcerative Colitis or Crohn’s Disease.

These booklets have been produced through the collaboration of NACC and CICRA, BSPGHAN, the IBD Section of the BSG and the Colitis and Crohn’s Nurses Group within the RCN Gastroenterology and Stoma Care Forum.

Copies of all three booklets can be supplied free of charge to hospitals throughout the UK, thanks to educational support grants from Convatec Ltd., Ferrings Pharmaceuticals Ltd., Nestlé Healthcare Nutrition and SHS Nutricia Ltd
